Every patient encounter, every insurance claim, and every medical record is tied to an ICD-10-CM code. And every medical billing and coding certification exam — from the AAPC CPC to the NHA CBCS — tests your ability to navigate this system accurately and efficiently.

For many students, the ICD-10-CM manual is intimidating. It contains over 70,000 codes organized across 21 chapters, with complex guidelines, instructional notes, and sequencing rules. But here is the truth: you do not need to memorize the manual. You need to understand its structure and know how to use it quickly. The difference between a coder who passes the exam and one who fails is not the amount of information they have memorized — it is the speed and accuracy with which they can navigate the manual under time pressure.

The Structure of an ICD-10-CM Code

Every ICD-10-CM code follows a specific alphanumeric structure that tells you exactly what category of disease or condition is being coded. Understanding this structure allows you to decode any code you encounter, even if you have never seen it before:

Characters 1-3: The category — a letter followed by two numbers (e.g., J18 = Pneumonia, unspecified organism)

Character 4: The etiology, anatomic site, or severity (e.g., J18.0 = Bronchopneumonia)

Character 5: Additional specificity

Character 6: Further specificity or laterality (right, left, bilateral)

Character 7: Extension — used primarily for injuries, fractures, and obstetric conditions to indicate encounter type

The decimal point separates the category (characters 1-3) from the subcategory and specificity characters. Always code to the highest level of specificity available — using a less specific code when a more specific one exists is considered incorrect coding and can result in claim denial.

The Alphabetic Index and Tabular List

The ICD-10-CM manual has two sections that must be used together. The Alphabetic Index is organized by main terms and subterms. You always begin your code lookup in the Alphabetic Index, using the condition or disease as the main term. The Tabular List is organized numerically by code and contains the official code descriptions, instructional notes, and coding guidelines.

The cardinal rule of ICD-10-CM coding: never code directly from the Alphabetic Index alone. Always verify the code in the Tabular List to ensure you have selected the most specific code available and that all instructional notes — including “code also,” “code first,” “use additional code,” and “excludes” notes — have been followed. Failing to follow instructional notes is one of the most common coding errors and a frequent source of exam questions.

The Most Tested ICD-10-CM Guidelines

The official ICD-10-CM guidelines are published annually by the Centers for Medicare and Medicaid Services and the National Center for Health Statistics. They are tested extensively on certification exams. The most frequently tested concepts include:

Guideline

Key Rule

Principal diagnosis

The condition established after study to be chiefly responsible for the admission or encounter

Signs and symptoms

When a definitive diagnosis has not been established, code the presenting signs and symptoms

Combination codes

A single code that captures both a condition and its associated complication or manifestation

Laterality

Many codes require specification of right, left, or bilateral — always code the most specific laterality documented

7th character extensions

A = initial encounter, D = subsequent encounter, S = sequela — must be applied to all applicable codes

Sequencing of diabetes codes

The diabetes code is sequenced first, followed by the manifestation code

Neoplasm coding

Use the Neoplasm Table in the Alphabetic Index; code behavior (malignant, benign, uncertain) and site

Tabbing Your Manual for Open-Book Exams

On open-book exams like the AAPC CPC, your ability to navigate the manual quickly is as important as your knowledge of the guidelines. Before your exam, tab your ICD-10-CM manual by chapter. Use color-coded tabs for the most frequently tested chapters: neoplasms (Chapter 2), diabetes and endocrine disorders (Chapter 4), mental disorders (Chapter 5), diseases of the circulatory system (Chapter 9), respiratory diseases (Chapter 10), and injuries (Chapter 19).

Practice locating codes under time pressure until you can find any code in under 60 seconds. Time yourself. The CPC exam allows approximately 2 minutes per question — and many questions require looking up multiple codes. Speed is not optional; it is a core competency.

Master ICD-10-CM Before Your Exam

NMACA’s Medical Billing & Coding Exam Prep includes comprehensive ICD-10-CM training with practice scenarios, coding exercises, and a full proctored practice exam.
